ADULT
 C4 provides a wide range of behavioral health care for adults. This care includes brief psychotherapy and group services where the focus is on symptom/stress reduction and social/role functioning, intensive services for consumers at high risk of hospitalization who lack daily living skills or have social interpersonal difficulties, as well as services that emphasize the development of independent living, socialization and vocational skills.
 
 
Adult Outpatient Services: offers a wide range of counseling, therapy, and case management to individuals, couples and groups with a range of mental health symptoms and diagnoses to help them live in the community. Services are also designed for Spanish-speaking clients utilizing culturally appropriate treatment modalities whenever possible.

 Intensive Services: serves adults facing severe, chronic mental illness including frequent psychiatric hospitalizations, or those struggling with both substance abuse and mental illness who need flexible and intensive services. Case management, assistance with daily living skills, crisis intervention, individual and group counseling, advocacy and linkage, medication monitoring, and assistance with transportation are provided. The goals of intensive services are to reduce or prevent hospitalization, enabling indiduals to remain stable in the community.

Forsenic Case Management Services: re-integrates mentally ill adults who have been adjudicated Not Guilty by Reason of Insanity (NGRI) of an offense from the Forensic Units of Elgin Mental Health Center into the community. This is achieved by conducting weekly pre-discharge groups providing case management services, and consultation with Elgin and residential staff to prepare an acceptable discharge plan for presentation to the court at a conditional release hearing.

Pre-admission screenings (PAS): are evaluations conducted on individuals suffering from a major mental illness who are seeking placement in a nursing facility. This screening process insures that individuals are adequately assessed to determine the appropriate long-term care needed for services.

Community Integrated Living Arrangement (CILA ) provides supervised supportive services that promote residential stability for an individual who resides in his or her own home or in the natural family home.

Adult Crisis Services: emergency services are available to provide prompt therapeutic intervention to those experiencing a psychiatric emergency. Trained staff assesses the severity and urgency of the situation and endeavor to stabilize the crisis. Staff determines the need for further treatment and connects individuals to appropriate services. These services are provided in the office and in the community wherever they are needed. Staff are also available after hours through C4's crisis on-call system to respond to crises whenever they occur.

HIV/AIDS Mental Health Services: in partnership with the AIDS Foundation of Chicago (AFC), C4 provides counseling and case management services to individuals who have both a mental illness and an HIV+diagnosis. Clients receive primary mental health treatment services from C4 in addition to services that focus on the impact that a diagnosis of HIV/AIDS has on a person's life. Individual and group counseling and intensive support are also provided, as well as linkage to a variety of resources throughout the area, including alternative therapies, support groups, hospitals, doctors and medication reimbursement programs.

Adult Outpatient Services at Broadway (AOB) among the traditional therapies offered, psychosocial rehabilitation services are provided to support individuals, who are in recovery from a mental illness, to optimize their potential. Members of the program are expected to fully participate in maintaining their own recovery and to develop their own individualized treatment goals to aid their progress toward full productivity and self-fulfillment. The program offers a large variety of group services to systemically assist members who are in recovery to develop personal skills, vocational rehabilitation, peer support, and community development.

Representative Payee Services: provides money management services replete with three teller windows that simulate the environment of a banking experience. Case managers work in close conjunction with the clients to establish their monthly budgets. Clients also have access to a monthly report showing the status of their accounts. These services are not restricted to C4 clients, but are open for referrals to the community and other agencies without representative payee services.
