Serves adults facing severe, chronic mental illness including frequent psychiatric hospitalizations, or those struggling with both substance abuse and mental illness who need flexible and intensive services. Case management, assistance with daily living skills, crisis intervention, individual and group counseling, advocacy and linkage, medication monitoring, and assistance with transportation are provided. The goals of intensive services are to reduce or prevent hospitalization, enabling indiduals to remain stable in the community.
