provides substance abuse assessment, referral and intensive case management services to intact families referred subsequent to the birth of a substance-exposed infant. C4 partners with Lutheran Social Services, a child welfare agency, to provide comprehensive services to these families, facilitate the establishment of a care and protection plan for the children, and support the custodial parents in successfully completing substance abuse treatment and avoiding foster care placements.
