For adults who need hospitalization or have been hospitalized, C4 maintains relationships with Elgin Mental Health Center, Chicago Read Mental Health Center and other community Hospitals to facilitate post-discharge. This relationship includes follow-up after discharge, assessment, referral/linkage with appropriate services, advocacy and input into discharge planning.
For children and adolescents who need psychiatric screenings or hospitalizations, C4 maintains relationships with community hospitals city-wide to provide screening, assessments, hospitals monitoring, discharge planning and community linkages to facilitate quality after care.
C4 maintains a collaborative relationship between the Chicago Police Department and Chicago Lakeshore Hospital wherein C4 provides assessments, pre-screening, crisis intervention and referral services 24-hours a day, seven days a week to individuals who are experiencing a psychiatric crisis and are brought in by the police.|
