Employee Benefits Summary 2017
Vacation time is a benefit earned by full-time employees performing continous service during a 12-month period. Hours are accrued at the following rates:

|During 6th year and thereafter:||160 hours|
Eligible employees do not accumulate more than 160 hours at any time. Note: part-time employees earn vacation time but at a lesser rate.
Full-time regular employees accrue up to 96 hours per year of sick time. The total sick time benefit is capped at 320 hours. Note: part-time employees earn sick time but at a lesser rate.
Full-time regular employees who have completed the introductory period of employment are entitled to 2 Personal Days per calendar year. Note: part-time employees are not eligible for Personal Days.
The agency observes 9 holidays annually. They are:

Full-time regular employees are eligible for 40 hours training time per year. E-learning, in-house classroom trainings, and Lunch ‘n Learns are also available.
Full-time regular employees are eligible for 3 days bereavement time for immediate family members.
Jury Duty Time
C4 will pay an employee’s regular salary for jury duty.
Flexible Benefit Plan
C4's Medical Insurance Contribution
C4 provides $1,575 in credits (prorated each pay period for plan year) to employees eligible for health care coverage.
Coverage begins on the first of the month which directly follows completion of a 30-day waiting period. Individual or family health insurance is provided by Blue Cross/Blue Shield. C4 offers four plans to choose from (three HMO and one PPO options). All four plans offer Prescription Drug Coverage and a Vision Discount Program.
C4's Wellness Credit
C4 provides a credit of $25 per pay period to all eligible employees who participate in and complete the agency's wellness program.
C4 offers two Dental plans: Dental DMO (dental HMO) and PPO benefit plans, which are administered through Guardian Life, and an employee can elect individual coverage or choose to include spounse/dependents.
C4 offers vision insurance through VSP for employees and their family.
Eligible employees can make pre-taxed annual contributions up to $2,550 toward medically related expenses that are not covered under agency’s insurance plan.
Dependent Care FSA
Eligible employees can make pre-taxed annual contributions of up to $5,000 ($2,500 for married individuals filing a separate return) toward Dependent Care Expenses.
Other Insurance Plans
C4 provides life insurance at the rate of twice the employee’s annual salary, up to $200,000 at no cost to the employee.
Full-time employees are eligible for short-term disability after 6 months of employment and long-term disability after completing 12 months of employment. This benefit is administered through Guardian Life Insurance.
Tax Deferred Retirement Plan
C4 employees are immediately eligible to participate in a tax deferred retirement plan. Up to $18,000 plus an additional $6,000 for employees over age 50 may be invested in a C4 Plan.

Direct deposit of payroll is available.
Employee Assistance Program (EAP)
C4's Employee Assistance Program is a free service designed to offer confidential and professional help to employees and members of their families who need assistance with personal problems.
Domestic Partner Insurance
Domestic Partner medical and dental insurance is available.
